Comprehensive Psychology Assessment and Report

A comprehensive report which can be used to support NDIS reviews includes:

  • Background semi-structured interview. This comprises an interview with a psychologist to collect relevant information including family history, developmental and medical background, diagnoses, education, employment, living situation assessments, activities of daily living, interventions, social and formal supports.
  • Structured Clinical Interview for the DSM-5 (SCID-5). This is a structured interview using the Diagnostic Statistics Manual -5 as a guide to formulate diagnoses.
  • Daily living skills assessment. This is a functional assessment looking at areas including communication, mental state, wellbeing, mobility, self-care, and self-management.
  • World Health Organisation Disability Assessment Scale 2.0 (WHODAS 2.0). This is a general measure of disability that can be used across all conditions. It examines domains of cognition, mobility, self-care, getting along with others, life activities and community participation.
  • Support recommendations which are based on the client needs and goals.
  • Ensuring support recommendations fit NDIS criteria so that appropriate, reasonable and necessary support can be accessed.

Progress Report

A progress report serves to track therapy and NDIS goal progression. It may be completed by the treating psychologist or a counselor. Progress reports act as evidence to be submitted to NDIS plan reviews. A progress report Includes:

  • NDIS and therapy goals review. This involves an examination of the progress towards or obtainment of the individual’s therapy and NDIS plan goals.
  • Examination of the goals then allows us to make recommendations for future therapy and NDIS goals moving forward.
  • Exploration of the barriers to goal obtainment will also be conducted to allow for modification within interventions and recommendations for future plans.
  • Based on the examination of the participant’s progress, support recommendations will also be made for the new NDIS plan review.
